- Please note all on-line registrations must be paid with a credit card. If you are unable to pay with a credit card please submit a paper application and accompanying check. Section 3 registrations:
Include the following supporting documents with your application:
- Completed Product Application
- Copy of the manufacturer's market place label
- Registration fee ($250 per product, including ABNs). Make checks payable to the DC Treasurer.
NOTE: A $250.00 late renewal fee is charged for applications received after January 31st of the registration year. - Copy of the Notice of Supplemental Distribution of a Registered Pesticide Product (EPA form 8570-5) for companies distributing a registered product originally produced and registered by the basic registrant.
- Letter of authorization designating an agent to act on behalf of a company (Only required for first-time registrants).

25 b product Registration Requirements:
Please submit the following:

- A Statement of Formulation
- Efficacy Data- a complete description of the tests on which the claims of efficacy are based. The efficacy data must:
1.Support all label claims for the product
2. Efficacy data must be collected by an independent testing entity, at minimum a single blind-study design and comparison to relevant industry products.
3. Include a description the experimental design, materials and methods; including controls and replication, statistical procedures, results (controls on the same graph as treatment data) and interpretation of those results
- Contact information and names of investigators conducting the study
- The complete label and labeling of the pesticide product. The label must include:
a) Directions for use
b) Precautionary Statements
i. Keep Out Of Reach Of Children
ii. Signal Word
c) Statement of all claims being made
b. Storage and Disposal
c. Product Weight
i. percentage (by weight) of active and inactive ingredients
- A Statement of Formulation- to include sources of each active ingredient and inert ingredient (CAS #?).
- If a product contains 8.5 % or more of an active ingredient1, then it should include:
a. The signal word "Caution"
b. The statement "Keep Out of Reach of Children"
c. Precautionary language and instructions for appropriate protective eyewear and gloves
- Products intended for topical application to human skin that contain an active ingredient should include:
a. The signal word "Caution"
b. The statement "Keep Out of Reach of Children"
c. A dermal sensitization precautionary statement, including a prohibition against application to the hands of children, and use directions requiring adult supervision during application to children
- Any additional information may be requested in order to complete registration of this product.
