Lead Screening Guidelines

Below are some important documents related to lead screening, including the form required by the District of Columbia Student Health Care Act, called “Certificate of Testing for Lead Poisoning.”  District law requires health care providers and health care facilities to provide a child’s parent or guardian with a Certificate of Testing for Lead Poisoning when one is requested.  (See D.C. Code § 22-7303.6)  District law also requires the parent or guardian of each child under the age of six (6) to submit the Certificate of Testing for Lead Poisoning upon entry by the child “into a licensed day care center, Head Start or similar early childhood program, pre-kindergarten, kindergarten or first grade in a public or private school in the District.”  (See D.C.
